AccessOAP Support Sessions — Registered Appointments

Register for a 30-minute in-person appointment with a Regional Service Network Specialist from AccessOAP. Select an appointment time from the Registration drop-down menu. For appointments related to first-time registration with AccessOAP , families will be required to provide 3 documents as proof of age , residency , and diagnosis during the registration process. Please bring this documentation to your appointment. Please see below for acceptable documentation.
